The Residential Case Manager will be responsible in assisting with incoming inquiry phone calls and tours of the facility as needed. This role will include screening potential residents and assisting in paperwork with admission to facility, in additional to orienting new residents and family members. The Residential Case Manager will also coordinate transportation for residents to and from appointments, conduct initial and periodic evaluations of the needs of the residents and will also assist residents with adjusting to life in the facility. This role will include Marketing responsibilities in the community and at various organizations and key referral sources.
Shift: 8:00 am - 4:00 pm Qualifications
Bachelor's degree with major work in human services or service delivery, and one year of full time experience in the provision of services to a dependent adult population.
