The Care Coordinator is responsible for assisting with the treatment process of their client caseload. He/She facilitates and supervises the treatment planning process. The Care Coordinator serves as a liaison and representative for child, family, and outside agencies within Maryhurst. He/She coordinates services both within Maryhurst and in the community as well as monitors effectiveness and efficiency of services for Quality Assurance. The Care Coordinator interfaces with other community resources such as legal and medical systems, as well as connects all services within the Maryhurst system for individual clients (therapy, medical, psychiatric, education, spiritual, etc). The Care Coordinator also assesses client/familys needs and makes referrals to necessary resources. He/ She ensures that the client is in most appropriate, least restrictive setting and has clear goals toward permanency..Requirements:
Bachelors degree in Human Services or related field. Minimum one year experience working with family and childrens services or in a case management position. Ability to facilitate the treatment process and collaborate with the treatment team effectively. Must have strong organizational, assessment, assertiveness, and documentation skills.
